- A list of all extracurricular activities and/or academic programs
- Essay on topic listed above (approximately 750 words or less)

Rules of Entry:
- Applicant must be attending a high school (Grades 9-12) or vocational school in Ohio during the 2009-2010 school year to be eligible.
- Application must be filled out completely (Download form | Download Rules).
- Candidate must have a minimum 3.0 GPA.
- Contest begins September 6, 2009 and all entries must be postmarked no later than midnight on December 31, 2009. Winners will be selected by January 13, 2010.
- Winners will be responsible for any and all tax liabilities incurred with their prize. All federal, state, and local regulations apply. Void where prohibited by law.
- The judging will be held by the What2be sponsors consisting of area health care facility members.
- By entering, each winner agrees to the use and re-use of his/her name, photograph and other biographical information by Cox Ohio Publishing.
- The winners will be contacted by phone, mail or email and their names and photographs will be published in our Spring What2be publication and online at www.what2be.net . The names of the winners may be obtained by sending a self-addressed stamped envelope to: What2be.net Contest, Attn: Heather Blakely, 1611 S Main St, Dayton, OH 45409 or by emailing the request to hblakely@coxohiomedial.com .
- The scholarship will be paid directly to the winner. Payment of awards will be contingent upon receipt of verification that the applicant has met all of the eligibility requirements.
- Immediately family members of Cox Ohio Publishing are not eligible to win.
